If you need help with a non-urgent medical or admin request, you can now contact us online using Patient Triage (4-days for you to get a response)

PLEASE CONTINUE TO CALL US IF YOU FEEL YOUR NEED IS URGENT, our staff will collect detailed information from you which will be passed to a GP, the GP will then be able to provide advice and help, including allocating an urgent apppointment where this is necessary

When completing a routine form, please include information about who you would like to see and how you would like to be seen (come in to see a clinician in person or a telephone appointment). If you are completing the form for a relative or friend, then please ensure you have your friend or relative's consent to complete the form as we will be making contact with them to make the appointment. The appointment request will be cancelled if the patient does not want to speak with us.

We will contact you within 4-days with a routing request, so please keep your phone with you at all times as the clinician may choose to call you directly or may send you a text message with advice/ an appointment to come in and see us. If we cannot get hold of you on the phone, we will assume that you no longer need our help.

Please check any text messages you get from us carefuly as we may ask you to come in and see a clinician (rather than have a telephone call if we feel an examination is needed). Non-attendance at these appointments wastes precious NHS resources. Please call if you need to change an appointment date or time.

How does Patient Triage work?

It's easy for patients and practices

Patient Triage is managed by a technology company called AccuRx who provide communications software to healthcare providers across the NHS. It can be used for booking an appointment or for any other query. Patient Triage is recognised by the NHS and has passed the NHS Digital Capabilities assessment and allows registered patients to submit either a medical query or an administrative request to their GP practice.

It is a simple online form which takes a few moments to fill out.

  • Easy to access: no app, no account needed - simply submit a query directly through the practice website. It should only take a couple of moments to do.
  • Convenient: available Monday - Friday, (Excluding Bank Holidays), so you can send in your request at a time that suits you, rather than having to wait for the phone-lines to open.
  • Faster: the requests are simple to submit, so you don’t need to wait on hold on the phone.
  • Efficient: your request will be seen by the right person, first time.
  • Fairer: those who need appointments most will get them, rather than those who got through on the phones first.
  • Turnaround time: we will get back to you within 4 working days.
  • Continuity: you can request to be contacted by a specific GP or member of staff – if they are available we will do what we can to honour your request.

 

Pharmacy Referrals

We now have access to an experienced Pharmacy team at Smartpharm who are able to diagnose and treat many conditions, they are also able to prescribe antibiotics. We may therefore refer you to Smartpharm if appropriate.

Multi-disciplinary team

We are lucky at Clevedon Medical Centre to have access to a wide range of experienced professionals including a physiotherapist who is able to diagnose musculoskeletal problems, mental health practitioners and pharmacists (experiecned in managing blood pressure, cholesterol and more).

Minor Ilness

Patients contacting about a 'minor illness' (e.g upper respiratory tract infections, rashes, muscular/ joint aches or pains, simple ear infections or UTIs) will be provided with a minor illness appointment. These appointments are with Advanced Care Practitioners (who may be a nurse of a paramedic by background) or Physician Associates who are able to diagnose and prescribe; more comples cases will be discussed with a GP if necessary. This leaves GPs free to see more complex presentations. If you feel your problem is more complex please ask for a GP appointment.

GP appointments

Please complete an AccuRx Request for all routine appointments (we will contact you back within 4-days). This new system avoids the 'morning rush' which invariably results in patients waiting on the phone for long periods or in some cases, not being able to get through to us at all. It also enables us to allocate you the most appropriate appointment (telephone or face-to-face). 

The AccuRx system also allows us to give you an appointment with the GP you normally see. Healthcare is best delivered by the GP you know and trust the most, particulaly if your needs are more complex. If you have an ongoing medical problem, please make sure you ask to speak to the same doctor in order to maintain continuity of care.

We are also able to provide video consultations if needed, and can request photos from you through your mobile phone should you find this easier than coming into the surgery.

Please remember, if you feel your problem is urgent please always call the practice and ask for an urgent same day appointment.

Reception Team

We aim to answer calls as quickly as possible, however at busy times you may have to wait and your call will be put in a queue. If your call is not urgent please be advised to call us after 10:30 when we know the telephone lines will be less busy.

In case of a medical emergency dial 999 for an ambulance.

Please avoid phoning on Monday morning if possible unless you are requesting an appointment as this is our busiest day.

Chaperoning

A chaperone will be made available during any examinations if felt necessary by a clinician, patient, carer or guardians request.

  • Monday 07:30 – 08:30
  • Tuesday 07:00 – 08:30
  • Tuesday 18:30 – 20:00
  • Wednesday 07:00- 08:30

Extended Access Service

If you require an appointment on a Saturday, and we are not open, you may be offered an appointment at another surgery in the area. If you accept an appointment at another surgery we will enable the GP at that surgery to have electronic access to your records for your appointment.

GP appointments are now available in your area during the evenings and at the weekend. To book an appointment, contact your practice.

Home Visits

If possible please try to telephone reception before 10:30 if you require an urgent same day home visit.

Alternatively if your home visit request is routine, please complete an AccuRx request form and submit. We will get back to you within 4-days.

A doctor or nurse, or our paramedic may phone you back before making a visit, as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a  different type of clinician, or indeed arrange a hospital attendance.

Home visits are only available for patients who are housebound because of illness or disability.

Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at the surgery.

Emergency calls and urgent requests will be accepted at any time.

When the surgery is closed please select option 2 to leave a message to advise us you are cancelling your appointment. If for any reason this option is not available you can send a message via this website which will go directly to the administrator as an email.

Text Reminder Service

We have a texting service which allows you to receive confirmation and reminders about your appointments.

To have this service you will need to register by completing a consent form.
